What is Breast Reduction Surgery?

Breast Reduction Surgery is a surgical procedure which entails removal of skin, glandular tissue and fat in oversized breasts. This leads to reshaping of the breasts giving it a more pleasant shape and a proportionate bust size.

Types of Breasts with regard to shape

TEARDROP – In this type, the majority of the breast tissue will sit below the nipple.

BELL – It typically has a more slender build at the top of the breast with the nipple sitting fairly low.

EAST WEST – This is characterized by the nipples pointing outwards towards the sides of the body rather than straight ahead.

SIDE SET – The majority of the breast tissue tends to drift toward the side of the body rather than the middle, thus side-set breasts, often referred to as wide-set breasts, typically have a greater space between them than other breast types. Like East West forms, Side Sets typically have a large center gap, but their breasts are bigger and their nipples look more forward than outward.

SLENDER – Slender breast shapes are characterized by having narrower width compared to round or full breasts and long, with less volume at the top compared to the bottom, and nipples that tend to point downwards creating a long and slender appearance.

ROUND – Breasts characterized by a uniform and evenly full appearance with the distribution of breast tissue being equal at the top and bottom

FULL – The breasts are close together and are rounder compared to the other forms. The distribution is even at the top, bottom and sides with the nipples pointing forward or up. A common finding is spillage or digging under the arms or breasts.

SHALLOW & SLENDER – These breasts are characterized by a wider base and a flatter profile, with the breast tissue spread out over a larger area compared to projected breasts. They are often slimmer at the top and fuller at the bottom, potentially appearing longer than wide. 

ATHLETIC – The chest shapes of athletes are characterized by a broader, more muscular appearance with the breasts having less soft tissue and a smaller cup size compared to other breast shapes, thereby appearing wider and flatter. 

CLOSE SET – These are characterized by breasts with most of the breast tissue lying in the center such that there is very little space between the breasts with a prominent cleavage. There is a large distance between the breasts and the shoulders.

ASYMMETRIC – Here the two breasts might differ in size, shape or position and this is extremely common. This might be more noticeable during puberty or after pregnancy.

CLASSIFICATION

Classification of Breast Hypertrophy

Breast Hypertrophy or Breast Enlargement is a pathologic condition which leads to oversized breasts in patients from one or many causes, primarily hormonal. Basically, it is classified into 2 types.

Macromastia

A condition where the weight of the excessive breast tissue is less than 2.5 kg.

Gigantomastia

A condition where the weight of the excessive breast tissue is more than 2.5 kg.

Back, Neck and Shoulder Pain

Excessive weight of the breasts can have a dragging effect and lead to back, neck and shoulder pain.

Pain in the Breasts

Mastalgia or pain in the breasts can arise in some patients with excessive size of the breasts.

Pressure from bra straps

The bra straps are subjected to excessive pressure from the huge weight of the breasts following which they may cut into the shoulders.

Skin Irritation

The undersurface of the breasts can get irritated especially when sweat accumulates in warm weather leading to rashes.

Infections or Abscesses

In extreme cases, the undersurface of the breasts can get infected and abscesses may develop.

Postural Problems

The dragging effects of the breasts leads to a stooping posture in patients which, in the long run, leads to spinal problems.

Sports & Exercise

The large size of the breasts may make it difficult for patients to participate in sports or performing exercises in the gym.

Clothing

It might be difficult for patients to find clothing that fit properly.

Nipple Numbness

With increasing size of the breasts, there is traction on the nipples such that the nerves supplying the nipples are stretched making them numb.

Skin Ulceration & Necrosis

The blood supply of the breasts may become tedious at certain areas and this, coupled with frequent rubbing from tight clothing, leads to ulcerations in the overlying skin.

Body Image Issues

Large breasts may lead to a loss of self-esteem and altered body image issues in affected patients.


Social Discomfort and Depression

Feelings of anxiety and shame are common and when coupled with ridicule, may lead to depression in individuals in the long run.

Diagnosis & Planning for Breast Reduction Surgery

Firstly, a proper history is taken including the time of onset of breast enlargement, any family history of breast enlargement, the symptoms experienced by the patient because of the breast hypertrophy and any additional concerns that you might be having. A medication and addiction history would also be taken including an allergy history.

Next, a proper examination is done. The size and shape of the breasts as well as symmetry is examined. The condition of the skin, the upper poles, nipple sensation, areolar size and position of the nipples with respect to the breasts are all noted.

Once, a diagnosis of breast hypertrophy is made, planning is made regarding breast reduction surgery. The incision lines, final scars and expected results are all informed to the patient. Any questions that the patient might be having regarding the surgical planning are all addressed at this stage.

Investigations

Routine investigations including blood tests, chest X ray and ECG are done in every patient and any abnormality is detected in any of these tests is corrected prior to surgery.
Apart from these routine investigations, a breast ultrasound in patients less than 35 years of age and a mammogram in patients older than 35 years is done to detect any other abnormalities in the breasts and to serve as a baseline against which further ultrasounds or mammograms may be compared.

Preparation for Surgery

Night Before Surgery

Have a light meal the night before surgery.
If you are feeling anxious, take an anxiolytic.
If you are taking any medications for blood pressure or any other problems you should take those unless prohibited by your surgeon.
Get a good night’s sleep!

Morning of Surgery

Do not take anything by mouth, not even water from the morning of surgery as your surgery would be done under general anesthesia.
You can have a bath if you feel like.
If you are taking any morning medications and have been informed by your surgeon to take the same then take it with a small amount of water.
Go to the surgical facility in time and get admitted and sign all the forms.

In the Operating Room

Markings would be done first and the final location of the nipples would be demonstrated to the patient.
Any final questions the patient might be having are answered at this stage.
The patient is next put under general anesthesia and surgery started.
The incisions are made, the excision is done and drains inserted and final closure done.

Post- Operative Instructions

You will need to stay in the surgical facility for 1 day and would be discharged home the next day after surgery.

Oral antibiotics would be given to you and you would need to take these so as to reduce any chances of infection.

Pain killers would be given to you along with antacids and these would be needed to reduce the pain which subsides drastically after the initial 2-3 days.

You would be advised to wear a sports bra for a couple of weeks after the procedure: this supports the breasts and lessens pain.

Drains would be removed after an appropriate time frame.

Take rest for the first few days after surgery. Bed rest is not recommended, ambulation is advised.

Avoid strenuous activities for around 2-3 weeks after surgery.

Clean the breasts 2-3 days after surgery with soap and water and then apply an antibiotic ointment and dressing as advised by your surgeon.

Follow-ups with the surgeon are recommended at appropriate time frames so as to assess healing, once the surgeon is assured that the wounds have healed well, he will then remove the stitches.

Scar lightening gels would be advised to you after the wounds have healed completely.

Similarly massaging your breasts would be of help once the wounds have healed.
