KEY POINTS:
– 3D Suction Assisted Liposuction results in a more sculpted body contour than conventional liposuction techniques.
– Multiple body areas can be targeted including neck, buffalo hump, arms, chest, back, abdomen, love handles, thighs, calves and so on.
– The process is safe if done within limits.
– Liposuction should not be thought of as a weight loss procedure, rather it should be regarded as a body contouring procedure.
– Small amounts of fat can be removed under local anesthesia but larger volumes would require spinal or general anesthesia.
What is Liposuction Surgery?
Liposuction is a surgical cosmetic procedure designed to eliminate excess fat deposits from targeted regions of the body, including the abdomen, hips, thighs, and buttocks. This technique employs a suction apparatus to extract fat cells, primarily for the purpose of body contouring and shaping and NOT for weight loss.
3D Suction Assisted Liposuction (3D-SLS)
3D suction-assisted liposuction, commonly referred to as 3D-SLS, is a method that effectively eliminates fat from both the superficial and intermediate layers of the skin. This results in a more refined body shape compared to traditional liposuction. It overcomes a drawback of standard techniques, which often leave behind superficial fat, potentially causing irregularities such as rolls and bumps after the procedure.
Target Areas for Liposuction
Liposuction can target stubborn fat in multiple areas of the body which would be discussed subsequently. However, it is important to discuss about a “360° liposuction” which can be beneficial for many patients.
Neck Liposuction and Double Chin Correction
Neck liposuction, referred to as cervicofacial liposuction, is a surgical technique designed to eliminate surplus fat from the neck and submental region, which is located beneath the chin. This procedure is frequently employed to enhance the aesthetic of a double chin and to achieve a more sculpted jawline. It entails the creation of minor incisions, the insertion of a cannula, and the application of suction to extract fat cells.
Anesthesia
Usually performed under local anesthesia.
Incisions
Small incisions are made under the chin and jawline and these are concealed.
Cannula Insertion
A small hollow cannula is inserted into the fat layers and fat suctioned out.
Suction
Suction is employed to remove the desired quantity of fat. Not all of the fat is taken out.
Sculpting
Sculpting is done by removing out the required amount of fat to achieve a more desirable and aesthetic contour. The ultimate goal is to produce a chiseled look like that of youth.
Post-operative
A chin strap is applied to help in reducing the swelling faster.
Buffalo Hump Correction
Treating a buffalo hump requires focusing on the root cause, which could entail modifications in lifestyle, adjustments to medication, or, in certain instances, surgical procedures such as liposuction. If the hump results from medication, a physician might recommend altering the dosage or transitioning to an alternative medication.
Adjustment of Medications
If the buffalo hump is the side effect of a medication then either the dosage needs to be reduced or the medication changed entirely.
Imbalances in Hormones
Conditions such as Cushing’s syndrome or adrenal tumors can lead to the development of buffalo humps. Treatment options may include medications, surgical intervention, or radiation therapy, tailored to the underlying cause.
Lifestyle Modifications
This primarily targets weight loss through exercise and diet to reduce fat deposits everywhere including buffalo hump.
Liposuction
Liposuction with the help of special small caliber cannulas can effectively remove fat from the buffalo hump.
Other surgeries
If the buffalo hump is the result of a tumor then it has to be addressed.
Non-surgical intervention
Sometimes, an injection can be injected into the buffalo hump thereby destroying the fat. This, coupled with neck strengthening exercises may help in removal of the buffalo hump.
Arms
Arm liposuction, commonly referred to as arm lipo, is a cosmetic surgical procedure aimed at eliminating surplus fat from the upper arms to enhance their shape and overall look. This minimally invasive technique targets persistent fat deposits that often do not respond to diet and exercise efforts.
Purpose
– Reduction of localized fat deposits in both upper and lower arms
– Improvement in shape and contour of the arm so as to enable the patient to wear sleeveless dresses
– Addressing the loose skin which may also be flabby especially after massive weight loss where a concomitant brachioplasty may be combined with arm liposuction to improve contour
Anesthesia
Can be performed under local anesthesia alone if a simple liposuction is needed, however, general anesthesia may be needed if concomitant procedures are added to arm liposuction.
Procedure
Removal of fat by making small slits in upper and lower arms and inserting cannula to aspirate out fat after injection of a special fluid.
Post-operative Advice
Compression sleeves are applied in the area of liposuction to reduce swelling and pain and to assist in adaptation of the loose skin to the underlying muscles.
Benefits
– Enhanced arm definition and form
– Refined and chiseled look of the arms
– Minimized sagging and excess skin
– Boosted self-esteem.
Recovery
– Short recovery time and patients can resume light work from the next day itself
– Some amount of swelling and bruising may be seen but these are quite common and resolve by themselves within a few weeks’ time
– Recovery is facilitated by wearing compression sleeves.
Important Considerations
– This method is not a solution for weight loss
– One must be realistic about the results as the bulk of the underlying muscles will also determine the final results
– Minimal scarring is produced as a result of this surgery
– If the skin is excessively loose then arm liposuction has to be combined with arm lift or brachioplasty to ensure good results.
Breasts
Breast liposuction, often referred to as “scarless breast reduction,” is a surgical technique that employs liposuction to eliminate surplus fat from the breasts, thereby decreasing their size and creating a more harmonious and proportionate look. This method serves as an alternative to conventional breast reduction surgery, especially for those who have primarily fatty tissue and limited sagging.
Highlights
– Minimally Invasive – only small slits are made to suction out excess fat from the breasts
– Target is fat – unlike breast reduction which targets removal of both glandular and fatty tissue, breast liposuction focuses on removal of fat only
– Moderate results – do not expect results akin to that of a formal breast reduction
Which patients qualify for Breast Liposuction?
– Patients with higher proportion of fat in their breasts
– Those with minimal sagging of their breasts
– Patients who are extremely resistant to scarring in their breasts
– Those who desire to breastfeed in future.
Benefits of Breast Liposuction
– Moderate reduction in the size of the breasts so as to achieve a more proportionate look
– Minimal scarring compared to breast reduction surgery
– Faster recovery time and expediated healing when compared with breast reduction surgery
– Preservation of normal shape and sensation in the breasts
Important Points
– This is not a surgery for everyone as the criteria for choosing patients are quite rigid
– When compared to formal breast reduction, long term results may not be comparable as sagging can appear with aging and other hormonal changes.
Abdomen
Abdominal liposuction, commonly referred to as stomach liposuction, is a cosmetic surgical procedure designed to eliminate excess fat from the abdominal region, enhancing body shape and contour.
This minimally invasive technique employs suction to extract persistent fat deposits that are often resistant to diet and exercise efforts. While liposuction can contribute to achieving a flatter and more toned abdomen, it should not be viewed as a weight loss method; it is most beneficial for those who are at or near their ideal weight and possess good skin elasticity.
Purpose
Removal of localized fat deposits from the upper and lower abdomen, waistline thereby improving the shape of the tummy and body silhouette
Technique
Infiltration of fluids by making small holes and then aspirating out fat using hollow cannulas which are inserted through the same holes
Anesthesia
Local anesthesia when limited amount of fat is taken out and general anesthesia when the area targeted is wide or when liposuction of various areas of the body are done at the same time or when liposuction is combined with other procedures.
Post-operative
An abdominal compression belt is given to aid in recovery
Recovery
Recovery should be complete within a week although some pain, bruising and swelling may persist
Results
Results are usually good especially when there is no loose skin. An improved waistline is obtained with gross reduction of subcutaneous fat in the abdomen. Patients must realize that this process cannot alter the intra-abdominal fat which must be targeted through dieting and exercise.
Alternative Surgery
If there is quite a bit of loose skin or divarication of the recti or loss in skin elasticity then an abdominoplasty or tummy tuck is recommended. This may be combined with liposuction in a procedure known as “Lipoabdominoplasty”.
Abdominal Etching
Also known as “ab etching” or “six pack liposculpture”, abdominal etching is a surgical procedure involving liposuction where the appearance of the abdominal muscles is enhanced.
Love Handles
Liposuction for love handles, commonly referred to as flank liposuction, is a cosmetic surgery designed to eliminate surplus fat from the flanks, or love handles, in order to achieve a more slender waistline and enhance body shape.
Fat in this area is interspersed with fibrous tissue and is much more resistant to weight loss or exercise so this is a commonly requested area for liposuction.
Procedure
– Done usually under local anesthesia
– Special cannulas are used to suction out fat
– Compression garment is applied postoperatively
Benefits
– Sculpting of waistline and improved body contour
– Results are long lasting if the patient maintains appropriate diet and is into an exercise program
– It is a minimally invasive procedure and recovery is extremely rapid.
Thighs
Thigh liposuction is a cosmetic surgical procedure designed to eliminate excess fat from the thighs, enhancing their shape and contour. The process entails creating small incisions and employing suction to remove fat deposits. This treatment is particularly effective for addressing persistent fat that is resistant to diet and exercise, leading to slimmer and more defined thighs.
Procedure
The target areas are first marked out and injected with a tumescent fluid followed by liposuction of the areas using cannulas.
Anesthesia
Done under local, spinal or general anesthesia.
Areas targeted
– Outer thighs
– Inner thighs
– Saddlebags (upper outer thigh)
Benefits
– Sculpted thighs
– Proportionate lower limbs
– Patient can wear skirts and jeans with confidence
Recovery
– Usually within 2-3 weeks the swelling, bruising and pain disappears
– Compression garments used during this period helps in recovery
Results
Final results would take around 6 months to appear because fat loss following liposuction is a continuous procedure.
Adjunctive Surgery
Can be combined with a thigh lift.
Calves
Calf liposuction is a cosmetic surgical technique that targets the removal of excess fat from the calf region, helping to reshape and slim the lower legs. This procedure can be applied to both the calves and ankles and is commonly viewed as a segment of a larger liposculpture process. It involves creating small incisions, using a cannula to break down fat cells, and then suctioning the fat away.
Procedure
Infiltration of tumescent fluid followed by liposuction through small slits using hollow cannulas.
Anesthesia
Local, spinal or general anesthesia
Benefits
Can aid individuals experiencing lipedema or other fat-related concerns in boosting their mobility, facilitating movement, and increasing their confidence.
Risks
– Swelling
– Bruising
– Heaviness of calves in the postoperative period
– Infection
– DVT if not mobile
– Tightness of skin
Recovery
Usually within 2 weeks, compression stockings are used to aid in recovery
Adjunctive Surgeries
Calf reduction surgery which remove part of calf muscles and/or nerve blocks to denervate the muscles or injection of botulinum toxin.
360° Liposuction
Also known as Lipo 360, 360-degree liposuction is a cosmetic procedure aimed at removing excess fat from the entire midsection, which includes the abdomen, flanks, lower back, and hips. This extensive approach yields a more sculpted and balanced figure compared to standard liposuction, which focuses on isolated areas. The “360” denotes the procedure’s comprehensive nature, targeting the full waistline.
Key Points
– Comprehensive Removal of Fat – The midsection is targeted from all aspects and fat is removed from the front, sides and back incorporating back rolls and love handles as well as the abdomen.
– Ultimate Body Contouring – This is a form of ultimate body contouring since it addresses the midsection from all aspects resulting in a more defined and toned appearance of the midsection.
– Target is a sleek silhouette – A more streamlined and proportionate body shape is achieved with this procedure.
– Combination Surgery – Patients having too much skin and fat excess in their midsection can be considered for an abdominoplasty along with Lipo 360 to achieve the best possible results.
Benefits
– Improved body aesthetics: This approach can enhance the visual appeal of the midsection, leading to a more balanced and contoured silhouette.
– Removal of stubborn fat: 360-degree liposuction is adept at eliminating persistent fat deposits, including love handles and back rolls.
– Increased self-assurance: A considerable number of patients indicate a rise in confidence and satisfaction with their body image after undergoing the procedure.
Remember
This is NOT a weight loss procedure, rather a body contouring procedure.
Risks of Liposuction
Just like any other surgery, there are certain risks associated with liposuction and the patient must be willing to take these risks before he or she is subjected to liposuction. Risks of the procedure include –
– Bleeding – Liposuction is a blind surgery and there might be injuries to blood vessels which may manifest as bleeding underneath the skin in the postoperative period.
– Reaction to anesthesia – Reaction to any form of anesthesia is quite rare but possible.
– Contour irregularities – may manifest as a bumpy, wavy, or sagging appearance of the skin, resulting from uneven fat removal, inadequate skin elasticity, and scarring. These alterations can be permanent.
– Fluid accumulation – can lead to temporary seromas, which are pockets of fluid that may develop beneath the skin and might require drainage via a needle.
– Numbness – in the treated areas can occur, either temporarily or permanently, and the surrounding nerves may experience irritation.
– Infections – Although skin infections are uncommon, they can occur, and a severe infection may pose a life-threatening risk.
– Injury to deeper structures – In rare cases, if the thin tube utilized during the procedure penetrates too deeply, it could puncture an internal organ, necessitating emergency surgery for repair.
– Fat embolism – is another serious concern, where fragments of fat may dislodge and obstruct a blood vessel, potentially traveling to the lungs or brain, constituting a medical emergency.
– Heart and Kidney Problems – When significant volumes of liposuction are performed, fluid shifts can lead to potentially life-threatening complications affecting the kidneys, heart, and lungs.
– Lidocaine Toxicity – Lidocaine, a medication used for pain management during liposuction, is generally safe; however, toxicity can occur, leading to severe issues with the heart and central nervous system.
– Death – The likelihood of complications and even death increases if the surgeon operates on larger areas of the body or conducts multiple procedures in one session resulting in a high volume liposuction.
Preparation for Liposuction
Prior to the surgery, it is essential to have a conversation with your surgeon regarding what to anticipate during the procedure. Your surgeon will evaluate your medical history and inquire about any existing medical conditions. It is important to inform the surgeon about any medications, supplements, or herbal products you are currently using.
Your surgeon may advise you to discontinue certain medications, including blood thinners and nonsteroidal anti-inflammatory drugs (NSAIDs), at least one week prior to the surgery. Additionally, you might be required to undergo specific laboratory tests before the procedure.
If the amount of fat to be removed is minimal, the surgery can typically be performed in a clinic or medical office. However, if a significant volume of fat is to be extracted or if you are undergoing multiple procedures simultaneously, the surgery may need to be conducted in a hospital setting. In either scenario, it is advisable to arrange for someone to drive you home and to stay with you for at least the first night following the procedure.
What to expect during the procedure
Before the procedure
Before undergoing liposuction, your surgeon may draw circles and lines on the areas designated for treatment. Photographs may also be taken to allow for a comparison of results before and after the procedure.
The method by which your liposuction is performed will depend on the specific technique chosen. Your surgeon will select this technique based on your treatment goals, the area being treated, and any prior liposuction experiences you may have had.
During the procedure
Certain liposuction techniques involve the administration of medications to numb the targeted area, known as local or regional anesthetics. If you receive local or regional anesthesia, you may also be given a sedative through an IV to help you remain calm and relaxed.
Alternatively, some procedures may utilize general anesthetics, which induce a sleep-like state referred to as anesthesia. Throughout the procedure, the surgical team will monitor your heart rate, blood pressure, and blood oxygen levels. If you are under local anesthesia and experience pain, it is important to inform your surgeon, as adjustments to the medication or procedure may be necessary.
The duration of the procedure can extend to several hours, depending on the volume of fat being removed. If you have received general anesthesia, you will awaken in a recovery room. Typically, you will remain in the hospital or clinic for several hours to allow your medical team to monitor your recovery.
In cases where a significant amount of fluid has been removed, you may need to stay overnight to ensure you do not experience dehydration or shock from fluid loss.
After the procedure
Anticipate experiencing some discomfort, swelling, and bruising following the procedure. Your surgeon may prescribe medications to alleviate pain and minimize the risk of infection.
Post-procedure, the surgeon might either leave your incisions open or insert temporary drains to facilitate fluid drainage from your body. Most patients are required to wear compression garments after surgery to help mitigate swelling; these garments are typically worn for several weeks.
You may need to take a few days off before returning to work, and it could take several weeks before you can resume your regular activities, including exercise. During this recovery period, be prepared for some changes in shape as the remaining fat adjusts. It may take weeks to months for the swelling to subside and for you to observe the final results.
Results
After undergoing liposuction, patients usually observe a reduction in swelling within a few weeks. By this stage, the treated area should appear less voluminous. Over the following months, you can expect the area to take on a slimmer appearance.
While aging can lead to a decrease in skin firmness, the results of liposuction tend to be long-lasting if weight is maintained. However, if weight is gained post-procedure, the fat distribution may shift. For example, fat may accumulate around the abdomen, irrespective of the areas that were treated.
